Elder Adult Substance Abuse Prevention   (back to top)

Three-session presentation throughout Pinellas County to groups of seniors with referrals to intervention programs when needed. Annual number of clients served: 1000 duplicated.


Older Americans Act Title III-B Counseling    (back to top)

In-home or in-office Mental Health (for persons with severe and persistent mental illness, provided by State of Florida Licensed Mental Health Counselor) or Gerontological Counseling (for persons experiencing emotional/situational difficulties utilizing a case management approach, provided by Masters Level Counselors and Bachelor level [or equivalent] volunteers).

Educational talks are provided to persons attending congregate dining sites to inform participants of mental health issues, coping skills, etc. This service is provided to South Pinellas County only; another agency provides this service to Central and North Pinellas elders. Annual persons served by Suncoast: 175 individual counseling and 850 unduplicated at dining sites and other group settings.


Community Care for the Elderly Case Management Programs   (back to top)

Provision of case management services and service authorization following assessment to persons throughout Pinellas County who are at risk of nursing home placement.

Community Care for the Elderly (CCE)
and Aged and Disabled Medicaid Waiver Programs

CCE Programs are the largest, serving 350 persons annually each, providing numerous services so that persons will remain in their location of choice.

Home Care for the Elderly serves 70-90 persons annually who require 24-hour supervision by caregiver(s) and provide financial assistance plus supplies and respite reimbursements when needed.

Alzheimer’s Disease Initiative serves persons 18 and above diagnosed with memory loss causing them to be at risk for nursing home placement; program serves 40 persons annually.

Assisted Living Medicaid Waiver serves persons at risk of nursing home placement and residing in assisted living facilities throughout Pinellas County; program serves 225-275 annually, and this is generally the frailest group with the most intense services of all.

Additionally, Suncoast Senior Support Programs may link with other Suncoast programs as needed, including: Adult Outpatient services and Adult Medical services for provision of medications, day treatment services, homeless services, the crisis response team for emergency services and substance abuse intervention programs.
